Amazon Tightens Engineering Controls After AI Tool Triggers Major Disruptions

Intelligence: Amazon announced a 90-day code safety reset across 335 Tier-1 systems directly tied to customer retail experience, after a series of AI-related outages including one triggered by its own AI coding tool Q resulted in approximately 1.6 million errors and 120,000 lost orders on March 2, followed by $6.3 million lost orders on March 5 from a change deployed without proper documentation.

  • Engineers working on critical systems must now get two peer reviews and complete documentation through an internal approval tool before any coding change is deployed, with automated checks enforcing Amazon's central reliability engineering rules at every step.

  • The root tension Amazon is navigating is that AI coding tools are non-deterministic, capable of producing slightly different outputs to identical inputs, which breaks corporate workflows that expect 100% consistency, making traditional software review processes an inadequate safety net on their own.

  • Amazon's fix combines agentic AI tools with deterministic, rules-based safeguards to create more predictable outputs, with the company explicitly acknowledging the reset is a temporary friction layer while it builds more durable long-term solutions.
